Taking up a month after where the last episode left off, we find Lex Luthor in the Belle Reve Sanitarium with three of Clark's old enemies, including the Kryptonite sniper from the episode "Extinction", the guy with the ability to become two individuals from the episode "Dichotic", and the temporary teen of steel who had accidentally stolen Clark's powers from the episode " Leech".

Jesse Metcalfe as Van McNulty (Kryptonite Killer)

Shawn Ashmore as Eric Summers (Teen of Steel) Best known as Iceman from the X-men movies.

Jonathan Taylor Thomas as Ian Randall (Twin Boy) best known as the kid from Home Improvement

Clark is finally able to visit Lex in the sanitarium where Lex begs Clark to get him out, but Clark feels he can't. Lex makes a futile attempt to escape, which only makes things worse for him.

Lana has to rehabilitate her legs after the last episode's horse trampling accident. At the hospital, she meets Adam, a fellow who turns out later to be a very mysterious character.

Finally, Clark decides to break Lex out of the sanitarium but he falls into a trap set by the three evil stooges. Using kryptonite gained by Van McNulty, Ian Randall beats Clark while Eric Summers plans to recreate the accident that originally gave him Clark's powers. At the same time Lex gets strapped in for a brain melting, courtesy of Lionel Luthor. Lionel hopes to reprogram Lex to forget Lionel's past murder of his parents.

Lionel is successful in removing recent memories from Lex including that of seeing Clark's powers in action from the last episode "Shattered". Clark is too late to stop Lionel, but manages to defeat his old foes.

Lex goes back to the mansion clueless about his father's cruel plot against him, while Lana gets closer to the mysterious Adam.  Lionel shows obsession over a video tape of Clark and Lex in the hospital where Lex tells Clark he knows his secret.  Clark goes to a party for Lana, but the two don't even speak to each other.
